Your health plan includes a specific preventive service schedule with recommendations for health screenings. Highmark closely follows preventive screening schedules for adults that are based on recommendations from the U.S. Preventive Services Task Force, the U.S. Centers for Disease Control and Prevention, and the American College of Obstetricians and Gynecologists.
If you are going to your doctor for a routine checkup and the doctor prescribes testing when you are asymptomatic (not experiencing symptoms), we strongly recommend that you talk with your doctor. Discuss Highmark’s preventive schedule and determine if/how the prescribed testing is listed on the schedule. If the doctor indicates that it is his/her typical practice to order testing not listed on the preventive schedule, then you should review with your doctor your financial responsibility for the prescribed testing.

Teamsters' 2015 Wellness Incentive
Being healthy means knowing your risk factors, understanding them and doing something about them. To encourage Teamster members to minimize health risks and gain better health, you have the opportunity to earn $100 in 2015 when you see your doctor to obtain a voluntary biometric screening and complete the Biometric Screening Attestation Form. Your doctor will certify that you have had the following services:
- full lipid profile (total cholesterol, HDL, LDL, and triglycerides)
- blood sugar
- blood pressure
- measured height, weight, calculated Body Mass Index (BMI)
After you and your doctor complete the attestation form, please send it to the Employee Benefits Division, as outlined directly on the form. You may submit your form anytime throughout the year, but the deadline to sumbit your form is December 15, 2105. Your reward of $100 (less taxes) will be processed through payroll and deposited into your paycheck within two months of submitting your completed form.
Keep in mind that any office visit copayment, deductible, or cost for form completion will be your responsibility. Results from the biometric screening should be discussed with your doctor and acted upon accordingly.
